FREE LANGUAGE ARTS LESSON - “Literacy Center Student Evaluation Form”

This is an easy-to-use student evaluation form for literacy centers. Students are held accountable for center work and reflect on their performance and participation.

This tool has the five days of the week listed with three different smiley faces to indicate their participation. I copy them front to back and cut them in half so they can use it for two weeks (and to save paper). It also is a nice tool for them to have so they know what centers they have fully completed. Two versions are included (because we love different fonts).
